New Vision has developed a Diabetes Education Program under the guidance of a Nurse Practitioner, Dietitian, Social Worker, Administrator, Nurse, Clinical Pharmacist and Physician. The goal of the program is to prevent or decrease the incidence of complications (both mental and physical) associated with diabetes through patient education and ongoing monitoring.
Our goal is to provide education to patients with newly diagnosed or long-standing type 2 diabetes or pre-diabetes. We also aim to provide regular follow-up and monitoring, while also promoting self-management, according to clinical practice guidelines.
In February 2008, patients began benefiting from educational group sessions and individual follow-up sessions in a collaborative manner involving each discipline. All patients with newly diagnosed type 2 diabetes, poorly controlled type 2 diabetes or pre-diabetes are referred to the Diabetes Education Program. Group sessions are led by the Dietitian (Certified Diabetes Educator), who discusses basic lifestyle management strategies for diabetes. Patients are followed by individual appointment with the Nurse Practitioner and Dietitian within one month of the group session. On-going follow-up is provided until the patient is stable. When healthy target levels are attained, the visits will be at three month intervals, alternating between the Nurse Practitioner and physician.
The Diabetes Education Program is also screening patients for depression due to the high comorbidity between diabetes and depression. Using the PHQ9 (Patient Health Questionnaire) we are identifying patients that may require mental health intervention and additional support, as evidence shows that depression and stress can be a barrier to good blood sugar control.
Patient satisfaction surveys are distributed after each group session in order to obtain patient feedback to ensure we are providing successful and beneficial educational sessions.
