Diabetes Education Program

Our Diabetes Program provides education, tools and ongoing support for patients with Type 2 diabetes and pre-diabetes. Our Diabetes Education Program focuses on patient self-management and strives to make the patient an active participant in their health care.

The program consists of an initial group education session, which focuses on healthy eating, physical activity and blood sugar monitoring. Following the group session, all patients are seen by the Nurse Practitioner and Registered Dietitian for individual appointments. Once blood sugars are under control, ongoing education and support is provided every 3-6 months by the diabetes team.

Patients with Type 2 diabetes and pre-diabetes.

Appointment Information

What you can expect at your appointment with your Registered Dietitian and/or Nurse Practitioner:

  • Your health and diet history will be collected and reviewed
  • Your weight and blood pressure will be taken
  • We will review your medications and discuss use of your blood glucose monitor
  • We will explain your recent diabetes blood work results
  • You will receive information about your chronic condition, use of your medications, learn more about healthy nutrition and gain confidence in your ability to manage your diabetes
  • You will set some lifestyle goals to work towards managing your blood sugars.

Referral Process

Speak to your health care provider or any team member to request a referral to the Diabetes Education Program.
