DIABETES MANAGEMENT PROGRAM

Types of Patients Seen

Our Diabetes Program provides education, tools and ongoing support for patients with Type 2 diabetes, long-standing diabetes and pre-diabetes focusing on patient self-management. We strive to make the patient an active participant in their health care.

What to Expect

Patients are seen by the Nurse Practitioner and Registered Dietitian/Certified Diabetes Educator for individual appointments. Once blood sugars are under control, ongoing education and support is provided every 3-6 months by the diabetes team, which may include a Registered Nurse.

At your appointment with your Registered Dietitian and/or Nurse Practitioner, you can expect:

  • Your current health and diet will be reviewed
  • History will be collected and reviewed
  • Your weight and blood pressure will be taken
  • A review your medications and discuss use of your blood glucose monitor
  • Provide information about your chronic condition, use of your medications, learn more about healthy nutrition and gain confidence in your ability to manage your diabetes
  • Work together to set lifestyle goals to work towards managing your blood sugars
