Referral Process

Patients with Asthma or COPD (or suspected Asthma or COPD) must be referred by a health care provider.

Patients who would like to discuss quitting smoking can refer themselves.

Is your Asthma well controlled?

  1. Do you need your Reliever inhaler (usually blue) more than 3 times per week?
  2. Do you have asthma symptoms (cough, wheeze, chest tightness or shortness of breath) more than 3 times per week?
  3. Do you have difficulty exercising or playing sports because of your asthma?
  4. Do you wake up one or more nights per week because of your asthma?
  5. Have you missed any work or school in the past month because of your asthma?
  6. Do you ever have asthma flare ups?

If you answered YES to any of the questions, your asthma is not likely well controlled.  With proper management, Asthma can and should be well controlled.  Our Respiratory Therapists will provide individualized education about your Asthma and how to keep it well controlled.  People with Asthma should have an Asthma assessment that includes spirometry testing (if appropriate) at least once per year.  Please speak to your health care provider if you are due for an Asthma Assessment.

Are you at risk for COPD?

            Canadian Lung Health Test:

  1. Do you cough regularly?
  2. Do you cough up phlegm regularly?
  3. Do even simple chores make you short of breath?
  4. Do you wheeze when you exert yourself, or at night?
  5. Do you get frequent colds that persist longer than those of other people you know?

If you are age 40 or older and answer YES to any of the Canadian Lung Health Test questions, please speak to your health care provider about having a spirometry test done.  COPD is a manageable condition.  Our Respiratory Therapists will provide patients with education about managing COPD and slowing the progression of it.

Do you want to quit smoking but don’t know where to start?  Have you tried to quit in the past but were not successful?  We can help!  Our Respiratory Therapists offer individual counselling to help you quit in a friendly and supportive atmosphere.  Quit Smoking classes are also offered periodically, check our calendar of events to find the next group session.
