The goal of our Heart Function Clinic is to improve the health status and quality of life and improve patient education adherence and empower patients to take a more active role in their health. Under the guidance of Dr. George Heckman, Geriatrician and Heart Function specialist, our Clinical Pharmacist and Nurse Practitioner work together to improve the quality of life for patients with heart failure, optimize medication therapies, and empower patients to take an active role in their health.

Appointment Information

What you can expect at your appointment with the Nurse Practitioner/Doctor and Pharmacist

  • Your health history and any new health concerns will be discussed
  • We will review your current diet and lifestyle
  • We will perform a physical exam including weight and blood pressure
  • We will explain recent lab work
  • We may adjust medications based on lab work, physical exam and how you are feeling
  • You will receive information about heart failure, use of your medications, and learn more about healthy lifestyle choices that will help you gain confidence in your ability to manage your disease.

Referral Process

Patients must be referred to the Heart Function Clinic by a health care provider.
