Types of Patients Seen

Every day, people living in Ontario who suffer from dementia and other memory disorders go undiagnosed or under-treated. This leads to preventable emergency room visits and long wait times to see specialists, placing an undue burden on the individual, their families, and the health care system. MINT Memory Clinics are providing patients and their families with high quality, compassionate dementia care in primary care settings within their own community.

Our Primary Care Memory Clinic provides multi-disciplinary care at the primary health care level for patients with cognitive impairment. The goal is to maintain and improve the quality of life for patients and caregivers.

What to Expect

Within the domain of our Memory Clinic, each patient has in-depth Cognitive Screening through accredited tools by a Nurse Practitioner or Nurse. During the same appointment, a Social Worker meets with the patient's caregiver for further screening and support. Collaboration with the physician is carried out and results in a family meeting at the end of the one-hour evaluation.

Referral Process

To access care in our Memory Clinic, you will need to be referred by your family physician or nurse practitioner. All services are fully covered by the Ontario Health Insurance Plan (OHIP). There is no additional cost to accessing this service.
